Subject: Important Information Regarding Medicare and Medicaid Insurance Participation
Effective July 1, 2025, Summit Family Health will no longer be participating with any Medicare Advantage or traditional Medicare plans. We will also no longer accept Medicaid insurance.
We understand that this may require you to establish care with a new medical provider. Here are some options to consider:
Explore In-Network Providers:
You may wish to contact your Medicare Advantage or Traditional Medicare plan to find an in-network provider who can continue to meet your healthcare needs. Establishing with a provider who is employed by one of the local health systems (St Lukes or St Alphonsus) may be beneficial. For patients with Medicaid insurance, please contact the Medicaid office to be assigned to a new provider and clinic.Utilize Out-of-Network Benefits:
We will be adopting an “opt-out” status with Medicare. As such, we will still be available to see you in our office, order tests, initiate referrals and coordinate your care. However, we will no longer bill Medicare for the treatment we provide and payment for services rendered by our office will be expected at the time of service. We will also be able to see patients with Medicaid insurance on a fee-for-service basis. However, due to Medicaid policies, we will not be able to initiate Health Connection referrals, perform prior authorizations for services, etc.

If you are going to switch healthcare providers, please initiate your search immediately as it may take several months to schedule an appointment at a new clinic and we don’t want your continuity of care to be interrupted.
